America's PPO is a PPO network consisting of over 5,300 physicians, hospitals and other providers in North Dakota and South Dakota. In order to access providers in this network, please be sure the America's PPO or ARAZ logo appears on your health benefits ID card.

CAI assembles the best combination of PPOs for employees based on employee location and the providers at which employees have traditionally spent healthcare dollars. These PPOs allow employees to access the full spectrum of healthcare services (acute care hospitals, substance abuse facilities, mental health facilities, laboratories, rehabilitation centers, surgical centers, physicians, chiropractic services and other specialties) at discounted rates. Cofinity builds on the strengths of several organizations for the benefit of insurance companies, third-party administrators and health plan managers. The network consists of several well known regional PPOs including the Flora Health Network, Mountain Medical Affiliates, PPOM, and Sloans Lake Preferred Health Networks. All offer extensive network management experience, highly efficient and automated claims re-pricing and reporting services, and well-established provider relationships.

CHN (Community Health Network) is a PPO with providers located primarily in the Flathead Valley area of Montana. CHN providers consist of physicians, hospitals and other medical care professionals.

Evolutions Health Care System is one of the largest privately-held PPOs in the nation with more than 600,000 network providers, 4,100 hospitals and 23,000 ancillary facilities. Other features include Utilization Management, Imaging/Data collection, Provider Bill Negotiations and Hospital Bill Audit services to the claim payers. In order to access providers in this network, please be sure the Evolutions Healthcare System logo is printed on your card.

Express Scripts is a pharmacy provider network. It can be accessed only if your plan is participating in the Express Scripts pharmacy card. To determine whether your plan is participating in the Express Scripts pharmacy plan, please consult your medical identification card for an Express Scripts sticker, or consult your employer or Allegiance for that information.

First Choice Health is a group of physicians, hospitals, clinics and other health care providers located primarily in Washington and Idaho. If you will be traveling to Idaho or Washington for medical treatment, please consult the First Choice web site. Use of a First Choice provider may result in measurable savings on medical costs. In order to access providers in this network, please be sure the FCHN logo appears on your health benefits ID card.

A Preferred Provider Organization (PPO) based in Sioux Falls, SD, servicing South Dakota, Iowa, Minnesota, Wyoming, North Dakota, Colorado, Montana, Nebraska, Utah, and Idaho. Providers include physicians, hospitals and other medical care facilities. In order to access providers in this network, please be sure the First Choice of the Midwest Inc. logo appears on your health benefits ID card. Providers will need to submit claims directly to First Choice of the Midwest for claim re-pricing The address is located on the back of your health benefits ID card.

Health InfoNet is a PPO with access to providers throughout most of Montana as well as community-specific access in Wyoming, Colorado and western North Dakota. Health InfoNet includes both facilities (hospitals and clinics) and individual providers including physicians, therapists and other health care providers. A PPO for which access is limited to covered persons under the Children's Hospital Central California Employee Health Benefit Plan. Interplan includes both facilities (hospitals and clinics), and individual physicians, therapists and other health care providers. InterWest Health is a PPO with providers throughout Montana and Idaho. Interwest includes facilities (hospitals and clinics), physicians, physical therapists, medical supply retailers and a number of other medical providers. Midlands Choice has developed an extensive network of physicians and hospitals that are available to provide high-quality care to members of its preferred provider organization (PPO). The network includes more than 15,000 physicians and other licensed health professionals and about 200 hospitals. Service area includes: WY, CO, SD, NE, KS, MN, IA, MO, WI, and IL. In order to access providers in this network, please be sure the Midlands Choice logo appears on your health benefits ID card. Providers will need to submit claims to the medical claims address listed on the back of your health benefits ID card.

Multiplan logo is required on member health benefit card in order to obtain discounts. If your card does not have the Multiplan logo, please contact Allegiance customer service to have a logo sticker forwarded to you prior to your visit to these providers. This is a nationwide PPO consisting of physicians, hospitals, and other medical care facilities. Multiplan should be accessed primarily by those who wish to locate a PPO network hospital or clinic outside the state of Montana.

The North Idaho Health Network (NIHN) covers plan participants in the five northern Idaho counties of Benewah, Bonner, Boundary, Kootenai and Shoshone. This Network includes 5 hospitals and over 200 active physician providers. In order to access providers in this network the NIHN logo must appear on your health benefits ID card.

PPO USA is a growing, national dental PPO network with more than 35,000 dentists practicing in more than 56,000 office locations. The network includes providers in all 50 states. Access is limited to those groups with the Connection Dental logo on their identification card.

TLC Advantage, L.L.C. is a network in South Dakota. This Preferred Provider Organization offers participants access to a rapidly-expanding network of providers. When you choose a TLC provider, you will have access to negotiated, favorable rates for your healthcare. TLC works to create strong working relationships with both participants and providers. In order to access this network you will need the TLC logo on your health benefit identification card.
