Tier 1

The Tier 1 Network includes the NCH facilities, NCH Medical Group providers, and Obstetrics.

• W4U Tier 1 office visits are eligible for $35 primary care and $45 specialty co-pays.
• W4U Choice Tier 1 office visits are eligible for $50 primary care and $60 specialty co-pays.

Under W4U, all facility services (SURGERY, IMAGING, THERAPY) need to be performed at an NCH facility or an authorization will be needed prior to services elsewhere.

NCHMD.org TO FIND AN NCH MEDICAL GROUP PROVIDER, GO TO “FIND A DOCTOR” AND ENTER THE PROVIDER'S NAME OR SPECIALTY AND CLICK "SEARCH". NCH MEDICAL GROUP PROVIDERS WILL HAVE "NCH MEDICAL GROUP" BY THEIR NAME.

NOTE: PROVIDERS WITHOUT "NCH MEDICAL GROUP" BY THEIR NAME ARE NOT PART OF TIER 1 NCH MEDICAL GROUP BUT HAVE PRIVILEGES AT NCH. BE SURE TO CHECK COVERAGE ELIGIBILITY FOR THOSE PROVIDERS UNDER TIER 2, CIGNA OPEN ACCESS PLUS NETWORK.

The CIGNA Open Access Plus Network includes physicians, hospitals and ancillary providers through the United States.
